Реабилитация детей с гемобластозами на различных этапах противоопухолевого лечения

Аннотация:

Введение. Комбинированное химио-лучевое лечение детей с опухолевыми заболеваниями кроветворной и лимфоидной тканей зачастую усугубляется полиорганной токсичностью, что в ряде случаев может приводить к развитию двигательных и функциональных нарушений.

Цель. Целью данного литературного обзора является оценка влияния лечебной физической культуры у детей с гемобластозами на этапах противоопухолевого лекарственного лечения на показатели активности и двигательные функции на основе анализа показателей доменов Международной классификации функционирования.

Материалы и методы. В ходе работы был проведен поиск по 10 базам данных медицинской литературы за период с 2015 по 2025 г. с использованием данных 19 исследований: 11 рандомизированных клинических исследований, 2 контролируемых клинических исследования и 6 когортных исследований.

Результаты. В основную и группу сравнения вошли 508 детей с гемобластозами. Межгрупповые результаты рандомизированных клинических исследований и контролируемых клинических исследований подтвердили, что ЛФК положительно влияла на снижение уровня опухоль-ассоциированной усталости во время интенсивной и поддерживающей фаз химиотерапии, повышала показатели физической активности и силы, улучшала толерантность к аэробной нагрузке.
При длительном наблюдении в стадии ремиссии у пациентов с гемобластозами отмечена продолженная положительная динамика в компенсации и нивелировании двигательных и функциональных нарушений.

Обсуждение. В перспективе требуется проведение многоцентровых рандомизированных исследований с унификацией методов оценки функционального статуса по доменам МКФ, что позволит объективизировать влияние ЛФК на различные аспекты физического развития и психосоциального состояния детей с гемобластозами на различных этапах противоопухолевого лечения.

Выводы. Представленные результаты, полученные на основе анализа данных литературного обзора, свидетельствуют о положительном эффекте ЛФК в отношении функциональных и двигательных нарушений у детей с гемобластозами.
